The 2009 Children’s Ministries Registration Form will serve to register your child or children for Wee Adventure, Great Adventure or Samuel’s Call Sunday School classes and/or any of our children’s summer activities.  These forms are necessary to ensure the safety and well being of all of our children at St. Luke’s.  
Below are the two links for the 2009 Registration Form. You can print these out and return them to the Church Office (Attn: Children's Ministries) at 222 NW 15, Oklahoma City, OK  73103.  These forms are also available at the north and south ends of the Great Adventure.  If you would prefer to receive this form by mail, send request to Kim Parris.
